My CS Link
Secure Online Health Management Tool
My CS-Link is a secure online health management tool that can connect you to the personal health information of you and your family.
Parents, if you do not already have a My CS-Link account, you can sign up by clicking on the link below

Through the My CS-Link Proxy Access, parents and caregivers of children 0-12 years old can access:
- Children’s immunization records
- After Visit Summaries
- Medications
- Test results
- Upcoming appointments
Please note: A parent or caregiver must first have their own My CS-Link account.
To gain proxy access to your child’s account, please complete the form below, and return it to our office via email at contact@thepediatricgroupbh.com or bring it with you to your child’s next appointment. If emailing, include a photocopy of your driver’s license or government issued identification.
Teens 12-17 years old can sign-up to access their Cedars-Sinai medical record from CS-Link. Teens have their own sign-in and password and can access My CS-Link from a smartphone or computer. Teens can decide to let a parent or legal guardian see parts of their medical record in My CS-Link via proxy access.
To request teen access to My CS-Link, teens can call our office to request a link to sign up via email. Once a teen has signed up for their account, they can request proxy access to their My CS-Link account for their parent or legal guardian by completing the form below and taking it to their next doctor visit or emailing it to our office at contact@thepediatricgroupbh.com AND calling office to give verbal consent.
